Seeking an experienced Nurse Practitioner for per diem home health assignments conducting risk assessments for Medicare Advantage plan members throughout Green County, WI. This role focuses on supporting individuals in Monroe, Brodhead, New Glarus, and neighboring communities, delivering invaluable health evaluations in the comfort of patients homes.

Schedule:

Flexible shifts available Monday through Sunday Set your own hours to match your availability Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

Complete annual and post-discharge home health risk assessments, each visit lasting approximately 45 minutes Review patients prior and current medical history, medical conditions, and medication use Provide clear education regarding health conditions and prescribed medications Assess for gaps in healthcare and make recommendations to optimize patient care

This position does not require developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license issued by the state of Wisconsin Current CPR certification (BLS) Board Certification through

AANP or ANCC Preferred:
