Participation in a weekend and holiday rotation is also required. The working hours when scheduled to work in the weekend and holiday rotation are 8am to 4:30pm. A day off during the week is given when working your weekend rotation or if your holiday rotation would have you scheduled to work more than 5 days in the work week. Hospice Alliance is seeking a compassionate RN to join our admissions team. In this role, you complete consultation and admission visits to educate and assess the patient's eligibility for hospice and palliative care services. Prior palliative care, home health and/or hospice experience and CHPN certification is desirable. Good nursing assessment skills, communication, critical thinking , flexibility and the ability to work independently and as a part of a collaborative team are all essential to being successful in this position. Travel to the patients place of residence to complete the admission or consult if required. Travel is primarily located within Kenosha and Racine counties and some portions of Walworth.
