Job Description

We are looking for a Long-Term Care RN to provide comprehensive nursing care within a long-term care facility. The ideal candidate will possess a strong clinical background, excellent assessment skills, and a commitment to enhancing the quality of life for our residents. As a LTC RN, you will be responsible for delivering personalized patient care, coordinating treatments, and ensuring compliance with healthcare standards.

8-13 weeks
Pay Rate:
$45.50 for local candidate Full-time options: 6a-230p or 2p-1030p, 5 days/week and every other weekend. Friday, Saturday, and Sunday doubles. Saturday, Sunday, and Monday doubles Part-time options: Every other Friday, Saturday, and Sunday 6a-230p or 2p-1030p Will take local candidate or traveler
Responsibilities:
Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation, to develop individualized care plans. Administer medications and treatments accurately Monitor residents' health status continuously, managing acute pain, infectious diseases, and chronic conditions Collaborate with interdisciplinary teams Maintain meticulous medical documentation within electronic health record (EHR) ensuring compliance with HIPAA regulations. Provide education and support to residents and their families regarding health coaching, medication administration, and disease management strategies. Ensure infection control practices are rigorously followed to prevent the spread of communicable diseases within the facility. Requirements Valid WI RN license 1 year of long term care experience within the last 2 years

From $45.50 per hour
