Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review prior and current medical history, health conditions, and medication use Educate patients on health management and medication adherence Identify care gaps and recommend appropriate follow-up actions Perform annual wellness or post-discharge assessments (no ongoing treatment planning or prescription required)

Qualifications:

Active Nurse Practitioner license in Wisconsin Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months recent NP experience preferred MSN degree from an accredited nursing program
