Sheboygan Responsibilities:

Conduct comprehensive assessments of high-risk members and develop individualized care plans in collaboration with primary care providers to improve health outcomes. Coordinate care across the healthcare continuum, including transitions of care, referrals, and collaboration with physicians, social workers, discharge planners, and community resources. Address medical and non-medical barriers to care, including housing, transportation, and access to services, while promoting treatment adherence, health education, and disease prevention. Maintain accurate clinical documentation, monitor outcomes, advocate for patients, and ensure cost-effective, high-quality care while protecting patient privacy and confidentiality. #
