Responsibilities:

Conduct comprehensive home health risk assessments, averaging 45 minutes per visit, for Medicare Advantage health plan members. Review and evaluate patients prior and current medical history, health conditions, and medication profiles. Deliver clear education on diagnoses, medications, and preventative health strategies during each assessment. Identify gaps in care and recommend actions or interventions to ensure optimal patient outcomes. Focus exclusively on assessmentno prescribing or treatment planning required.

Qualifications:

Active Nurse Practitioner (NP) license in Wisconsin Board Certification through AANP or ANCC Current CPR certification (BLS) Master of Science in Nursing (MSN) from an accredited institution At least six months of recent nurse practitioner experience preferred This position provides competitive pay per evaluation completed, plus a mileage stipend and compensation for administrative time.
