In this role, you will assess new clients for eligibility, complete OASIS assessments, obtain consents, and develop individualized care plans. You will collaborate with an interdisciplinary team to ensure high-quality, coordinated care in clients' homes. Key Responsibilities Conduct initial client assessments and complete OASIS documentation Develop and coordinate interdisciplinary care plans Educate clients and families on Home Health services Communicate findings with physicians and care team Ensure timely and accurate documentation Participate in care coordination, scheduling, and on-call rotations Qualifications Current Wisconsin RN license (BSN preferred) 1+ year of Home Health RN experience 1+ year of OASIS experience CPR certification Strong communication and assessment skills Valid Wisconsin driver's license, reliable transportation, and required insurance coverage Why Join Us?
