Join the St. Camillus Home Health team as an Admission Nurse, where you'll play a vital role in helping patients begin their home health journey with confidence, comfort, and personalized support. In this rewarding position, you'll be the first point of contact for new clients—conducting comprehensive assessments, developing individualized care plans, and collaborating with an interdisciplinary team to ensure the highest quality care in the place patients call home. What You'll Do Conduct admission visits and complete OASIS assessments and documentation Evaluate patient eligibility for Home Health services Develop and coordinate personalized interdisciplinary care plans Educate patients and families on services, expectations, and care goals Collaborate with physicians and care team members to ensure seamless care transitions Maintain timely, accurate, and compliant documentation Participate in care coordination, scheduling activities, and on-call rotation Participate in an on-call rotation What You'll Bring Current Wisconsin RN license (BSN preferred) At least one year of Home Health nursing experience At least one year of OASIS assessment experience Current CPR certification Strong clinical assessment, communication, and organizational skills Valid Wisconsin driver's license, reliable transportation, and required insurance coverage Why St. Camillus?
