Job Description

We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The ideal candidate will possess a strong background in patient care, with experience in long-term care, acute care, and specialized nursing practices. $28.23 - $41.00 per hour Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform individualized care plans. Administer medications and treatments accurately according to physician orders while adhering to safety protocols and HIPAA regulations. Monitor residents' conditions closely, recognizing changes that require intervention or escalation to medical teams. Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care strategies. Provide education and support to residents and their families regarding health conditions, medication management, and post-discharge instructions. Assist with procedures such as IV infusions, catheterization, wound care, and other advanced nursing interventions as required. Ensure infection control practices are rigorously followed to maintain a safe environment for residents and staff. Skills Proven experience working in skilled nursing facilities with familiarity in long-term care regulations and compliance standards. Strong clinical skills, including patient assessment, medication administration, IV infusion, and wound management. Experience with specialized care areas such as Alzheimer's/dementia care, memory care, hospice services, and behavioral health needs. Ability to perform triage effectively in urgent situations while maintaining calmness under pressure. Excellent communication skills for caregiver collaboration and client interaction across diverse populations, including geriatric patients and individuals with disabilities.
