Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members in their homes Review and evaluate prior and current medical, surgical, and medication histories Conduct post-discharge visits as needed Provide patient education on chronic and acute conditions, as well as medication regimens Identify care gaps and offer recommendations to improve health outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in West Virginia Current BLS (CPR) certification National Board Certification (AANP or ANCC) MSN from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner preferred This position offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and additional payment for administrative time.
