Schedule:

Full-time, Monday through Friday 13-week contract, with potential to extend On-call participation may be required Structured daily patient visit expectations

Essential Responsibilities:

Deliver skilled in-home nursing care and interventions for diverse populations Complete accurate patient assessments and promptly submit OASIS documentation Work collaboratively with physicians and interdisciplinary care teams to tailor care plans Provide wound care, medication management, and patient/caregiver education Monitor and report changes in patient conditions to the clinical team in a timely manner

Preferred Qualifications:

Active RN license in good standing Minimum of one year of recent home health RN experience Demonstrated proficiency with OASIS documentation (required) Strong familiarity with HomeCare HomeBase (highly preferred) Excellent organizational and time management skills to handle a consistent patient load Reliable transportation and valid drivers license
