Key Responsibilities:

Conduct comprehensive annual health risk assessments in patient homes Review prior and current medical history, chronic conditions, and medication usage Offer guidance and education regarding medical conditions and prescribed medications Identify gaps in patient care and recommend appropriate next steps Document findings accurately; no requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in West Virginia Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least six months of recent experience as a nurse practitioner preferred This position provides competitive per-evaluation compensation, with each assessment compensated at $80-$90, including a mileage stipend and additional administrative time.
