Responsibilities:

Conduct home-based annual risk assessments for Medicare Advantage members Review prior and current medical history, medical conditions, and medication use during in-home visits (approximately 45 minutes per assessment) Educate members about their medical conditions and medications Identify gaps in care and offer recommendations to optimize health outcomes Complete comprehensive documentation after each visit No treatment plan development or prescribing responsibilities

Qualifications:

Valid Nurse Practitioner license in West Virginia Board Certification (AANP or ANCC) BLS (CPR) certification MSN from an accredited nursing program At least 6 months of current Nurse Practitioner experience (preferred) Excellent interpersonal and communication skills Professional, compassionate approach to patient care in the home setting
