Job Description

Delivering compassionate care and clinical expertise, this position focuses on supporting patients and families throughout the hospice journey in home and residential settings. The individual in this role provides skilled nursing interventions, symptom management, and emotional support, ensuring comfort and dignity for those facing terminal illness.
Schedule:
Monday through Friday, 8:00 AM 5:00 PM Occasional weekend or on-call rotation may be required
Key Responsibilities:
Conduct thorough nursing assessments and develop individualized hospice care plans Deliver direct, skilled nursing care with an emphasis on symptom and pain management Administer prescribed medications, treatments, and interventions Monitor and document changes in patient condition; communicate updates to the care team and medical director Provide patient and family education on disease progression, comfort care, and medication administration Collaborate with interdisciplinary team members, including social workers, chaplains, aides, and volunteers Maintain timely, accurate, and thorough documentation using Home Care Home Base (HCHB) Attend case conferences, team meetings, and participate in quality improvement initiatives Adhere to established hospice policies, protocols, and regulatory standards Applicants should possess: Active RN license in state of practice (in good standing) Minimum one year of recent hospice experience Proficiency with Home Care Home Base (HCHB) documentation Strong clinical assessment and communication skills In-depth knowledge of end-of-life care and family support principles Reliable transportation, valid drivers license, auto insurance Ability to work autonomously in a field-based role Compassion, empathy, and emotional resilience Preferred qualifications include a BSN, experience in palliative/end-of-life care, and familiarity with Medicare hospice guidelines.
