Schedule:

13-week contract, full-time Monday through Friday On-call responsibilities probable

Key Responsibilities:

Deliver skilled nursing care directly in patients homes Complete comprehensive assessments and ensure timely, accurate OASIS documentation Coordinate with physicians and care teams to execute individualized care plans Perform wound care, administer medications, provide patient teaching, and monitor chronic conditions Update clinical teams promptly regarding changes in patient status

Preferred Qualifications:
