Job Description

Make a meaningful impact supporting patients and families during their most challenging moments. This role focuses on delivering specialized end-of-life care and emotional support in home and residential settings. As a registered nurse with expertise in home health and hospice, you will be part of a compassionate interdisciplinary care team, ensuring comfort, dignity, and quality for every patient.
Schedule:
Monday Friday, 8:00 AM 5:00 PM Occasional weekend or on-call rotation 13-week contract
Key Responsibilities:
Perform thorough nursing assessments, developing and implementing individualized hospice care plans. Deliver direct skilled nursing interventions for symptom relief, pain management, and comfort measures. Administer ordered medications and treatments, and promptly communicate any patient status changes to the care team. Offer education and support to patients and their loved ones on disease progression, medication, comfort techniques, and emotional resources. Work collaboratively with social workers, chaplains, nursing aides, and volunteers to meet the holistic needs of each patient. Accurately document all care and communications in Home Care Home Base (HCHB). Attend interdisciplinary meetings and participate in ongoing quality improvement. Uphold compliance with hospice guidelines, policies, and standards.
Qualifications:
Active, unrestricted RN license in the state of practice Minimum one year of recent hospice experience Proficient in Home Care Home Base (HCHB) documentation Excellent clinical assessment, organizational, and communication abilities Skilled in pain management and end-of-life care principles Reliable transportation, valid drivers license, current auto insurance BSN and additional end-of-life care experience preferred

The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals.
