You'll work Days, Evenings, and some Weekends with a rotating on‑call schedule. Responsibilities Complete comprehensive assessments, initiate care plans, and establish patient baselines. Evaluate patient status after hospitalization or interruption in services. Conduct timely reassessments to ensure continued eligibility and update care plans. Required Qualifications Active RN License in the state of West Virginia Strong clinical judgment and ability to work independently in the field Reliable transportation Home health experience
