Job Description

Deliver exceptional, compassionate hospice care as a Registered Nurse, supporting patients and families facing end-of-life transitions in homes and residential settings. This contract position emphasizes a patient-centered approach, ensuring comfort and dignity while collaborating closely with an interdisciplinary team.
Schedule:
Monday Friday, 8:00 AM 5:00 PM Occasional weekend or on-call rotation may be required
Key Responsibilities:
Complete thorough nursing assessments and develop individualized hospice care plans Deliver skilled nursing interventions for symptom management, pain control, and patient comfort Administer medications and treatments per physician orders and hospice protocols Monitor patient changes and promptly update the medical director and care team Educate families and caregivers on disease progression, comfort measures, and emotional support Collaborate with social workers, chaplains, aides, and volunteers to meet physical, emotional, and spiritual needs Maintain accurate, timely documentation using Home Care Home Base (HCHB) Engage in case conferences, team meetings, and ongoing quality improvement initiatives Ensure full compliance with hospice policies, procedures, and regulatory standards
Qualifications:
Active RN license in good standing in the state of practice At least 1 year recent hospice experience required Proficiency with Home Care Home Base (HCHB) electronic documentation Strong clinical assessment and interpersonal communication abilities Solid foundation in pain management and end-of-life care principles Reliable transportation, valid drivers license, and auto insurance Independant, adaptable, and compassionate professional demeanor
Preferred Qualifications:
