Job Description

Delivering compassionate hospice care requires a blend of clinical expertise, empathy, and collaboration. This vital field-based position calls for an experienced Registered Nurse ready to provide exceptional end-of-life care and holistic support to patients and their families. The contract spans 13 weeks with a full-time scheduleMonday through Friday, 8:00 AM to 5:00 PMwith occasional on-call or weekend responsibilities.
Responsibilities include:
Conducting nursing assessments and developing individualized hospice care plans Delivering skilled nursing care focused on comfort, pain management, and symptom control Administering medications and interventions according to hospice protocols and physician orders Monitoring changes in patient conditions and coordinating timely communication with the medical director and care team Providing education and emotional support to patients, family members, and caregivers Collaborating with interdisciplinary teams (including social workers, chaplains, and aides) to address comprehensive care needs Accurately completing documentation in Home Care Home Base (HCHB) and ensuring compliance with all hospice policies and regulatory guidelines Participating in case conferences, team meetings, and continuous quality improvement Managing daily travel to patient locations within the assigned service area To thrive in this role, youll bring: An active RN license (in good standing) At least one year of hospice nursing experience Proficiency in Home Care Home Base (HCHB) software Strong clinical assessment, communication, and teaching skills Reliable transportation, a valid drivers license, and current auto insurance Resilience, compassion, and a commitment to high-quality end-of-life care BSN degree and prior palliative care experience preferred Benefits include weekly pay, comprehensive medical/dental/vision insurance options, 401(k) retirement plan options, 24/7 support services, and mileage reimbursement.
