Warm Valley Health Center is seeking a skilled and compassionate Clinical Nurse/Case Manager to join our fast-paced outpatient team in Fort Washakie, WY. What You'll Do:

Perform patient triage, assessments, and administer urgent care treatments Deliver medications, IV therapy, and assist in minor procedures Conduct EKGs, lab draws, and prepare patients for diagnostic exams Coordinate care plans and referrals with an interdisciplinary team Provide chronic care support, case management, and patient follow-up Lead wellness initiatives and community health education outreach Maintain accurate and timely EHR documentation, ensuring compliance

What You Bring:

BSN required Active Wyoming RN license BLS certification (ACLS/PALS preferred) Experience in outpatient, urgent care, or case management Certification in Case Management is a plus (or willingness to obtain within 1 year)
