This position works closely with the Client Coordinator and other staff to manage patient intake, coordinate services, and maintain high standards of customer service.

Key Responsibilities:
Capture and communicate patient referral information to management teams Process and document incoming patient needs, service requests, and changes Inform patients accurately about available services Collaborate with operations to schedule appropriate personnel promptly Assist with new case openings in patient homes Record and report patient and employee concerns to appropriate parties Enter patient data into the system of record per company policies Support office staff with routine administrative duties Perform other duties as assigned
Qualifications:
Ability to travel up to 25% High School Diploma or equivalent required; LPN licensure and/or college degree preferred Minimum of 1 year of relevant experience preferred Strong organizational and time management skills Excellent communication and customer service abilities Professional telephone manner and interpersonal skills Proficiency in Microsoft Office Suite (Word, Excel, Teams) Ability to lift up to 15 pounds
Compensation:
$27.00 per hour. $2,000 annual bonus potential.
