About Our Client Our client is an established home health organization committed to providing high quality, compliant, patient centered care throughout the communities it serves. They are seeking an experienced Clinical Director / Director of Patient Care Services (DPCS) to provide clinical leadership, regulatory oversight, and quality management across the organization. This is a key leadership position for an experienced home health professional who understands the importance of clinical excellence, regulatory compliance, documentation integrity, accountability, and responsive leadership. The DPCS will serve as the designated Clinical Manager under Medicare Conditions of Participation and maintain clinical oversight of patient care delivery, documentation, QAPI, and clinical staff performance. What You'll Be Responsible For In this role, you will: Provide overall oversight of direct and indirect clinical services Ensure compliance with Medicare Conditions of Participation, California regulations, and ACHC standards Serve as the designated Clinical Manager under Medicare CoPs Oversee individualized Plans of Care and ensure they are properly developed, implemented, and updated Monitor clinical documentation for accuracy, timeliness, and regulatory compliance Conduct clinical record reviews and audits Oversee clinical staffing and patient assignments Provide clinical oversight of admissions and determine clinical appropriateness Maintain accountability for documentation standards and clinical productivity Investigate clinical incidents, patient safety concerns, and quality issues These responsibilities reflect the clinical compliance and oversight requirements established for the position. OASIS, Documentation & Compliance The DPCS will play a critical role in maintaining documentation and reimbursement integrity, including: Overseeing OASIS assessment accuracy, validation, and corrections Reviewing OASIS scoring trends and outcomes Ensuring documentation supports medical necessity and PDGM reimbursement requirements Supporting ADR responses and external audit preparation Monitoring timely chart completion and EMR compliance Quality Assurance & Performance Improvement You will lead the agency's Quality Assurance and Performance Improvement (QAPI) program and help maintain ongoing survey readiness. Responsibilities include reviewing hospitalizations, infections, falls, incidents, complaints, and patient satisfaction data; developing corrective action plans; and preparing quality reports. Clinical Team Leadership This position requires a leader who can establish clear expectations and hold the clinical team accountable.

Hiring, supervising, evaluating, disciplining, and when necessary terminating clinical personnel Conducting annual and as needed performance evaluations Overseeing employee orientation and competency validation Providing in service and regulatory training updates Monitoring clinician productivity and visit utilization Reviewing overtime and staffing efficiency Adjusting staffing oversight based on patient census Partnering with leadership regarding clinical budget management The original position specifically assigns supervision, evaluation, competency validation, and regulatory training responsibilities to the DPCS. On Call & Availability Expectations Because this position plays an essential role in continuity of patient care, the DPCS must maintain consistent accessibility during operating hours and assigned on call periods. The individual must be responsive to urgent clinical situations, patient concerns, hospitalizations, after hours escalations, staff questions, physicians, referral sources, and organizational leadership. The DPCS will participate in the agency's on call rotation and provide clinical decision making and administrative oversight when assigned. Qualifications Current California Registered Nurse (RN) license required , or licensed physician in California BSN required Master's degree preferred Minimum 3 years of management experience in home health Strong knowledge of Medicare Conditions of Participation, ACHC standards, OASIS, PDGM, and California regulations Demonstrated experience leading QAPI and compliance programs Strong leadership, regulatory interpretation, clinical decision making, and communication skills What Our Client Is Looking For Our client is looking for someone who is not simply capable of managing clinical operations, but who takes ownership of them.
