Job Description

Position Summary:
Home Health Intake Coordinator We are a family-owned and locally operated home health agency dedicated to delivering compassionate, high-quality care to patients across California. We are currently hiring a Home Health Intake Coordinator to support our clinical and administrative team. This role is essential to our patient onboarding process and directly supports our mission of delivering seamless, compliant, and patient-focused care. The first 30 days will serve as a working interview period to evaluate fit, performance, and alignment with our agency's values.
Key Responsibilities:
  • Professionally handle all incoming referral and inquiry calls, providing clear and compassionate explanations of the home health assessment and intake process.
  • Complete all referral documentation accurately and efficiently using WellSky and other agency software systems.
  • Verify patient eligibility and benefits for Medicare, Medi-Cal, and commercial insurance plans.
  • Coordinate initial assessments with clinical staff, scheduling timely nurse or therapist visits per regulatory timelines.
  • Review referrals for completeness and compliance with California Title 22 and Medicare Conditions of Participation.
  • Ensure all intake documentation complies with federal and state regulations, including HIPAA, CMS, and California Department of Public Health (CDPH) standards.
  • Collaborate with discharge planners, physicians, and case managers to obtain signed plans of care and medical necessity documentation.
  • Maintain accurate and timely logs for referrals, admission status, and communication history.
  • Monitor and follow up on pending authorizations for services under managed care or commercial plans.
  • Assist in performance improvement initiatives by identifying referral barriers or delays in patient onboarding.
  • Maintain confidentiality of patient information and adhere to all HIPAA privacy laws.
  • Provide administrative support including faxing, emailing, insurance verification, and correspondence.
  • Use Microsoft Office Suite (Word, Excel, Outlook) for internal reports and documentation.
Required Qualifications:
  • Minimum 1 year of experience in a home health, hospice, or medical intake setting.
  • Strong understanding of Medicare/Medi-Cal eligibility and California home health regulations.
  • Proficiency in EMR systems (preferably WellSky) and Microsoft Office applications.
  • Strong communication skills with attention to detail and customer service orientation.
  • Ability to work independently and within a team in a fast-paced, deadline-driven environment.
Preferred Qualifications:
  • Bilingual in English and Spanish or Tagalog.
  • Familiarity with California Title 22, CDPH guidelines, and CMS regulations for home health.
