As a Patient Care Coach, you will work closely with clients, families, and interdisciplinary teams to deliver tailored interventions and ensure a safe, supportive environment. Duties Assist in developing and implementing individualized care plans based on behavioral assessments and client needs Support clients with daily activities, including skill-building exercises, behavior management strategies, and social interactions Conduct intake assessments and ongoing data collection to monitor progress and inform treatment adjustments Administer medication following prescribed protocols while maintaining accurate medical records in electronic health records (EHR) systems Utilize de-escalation techniques and crisis intervention methods to manage challenging behaviors safely and effectively Support clients with autism spectrum disorders, mental health conditions such as PTSD, substance use disorders, and other neurodevelopmental challenges Document all patient interactions thoroughly in compliance with HIPAA regulations and medical terminology standards Skills Experience working with children, youth, or individuals with disabilities or neurodevelopmental disorders such as autism spectrum disorder Knowledge of behavior intervention strategies including applied behavior analysis (ABA) and behavior management techniques Proficiency in electronic health records (EHR) management systems and medical documentation practices Strong understanding of mental health issues including PTSD care, addiction counseling, crisis management, and crisis intervention Ability to perform vital signs measurement, first aid, CPR certification, and maintain patient confidentiality at all times Excellent client communication skills with an emphasis on compassionate support and active listening Familiarity with medical terminology, medication administration procedures, and working within HIPAA guidelines Join us in making a meaningful difference!
