Principal Responsibilities:

Perform comprehensive social work assessments and interventions for inpatients and Emergency Department patients. Develop and implement measurable, realistic plans to improve social service delivery. Conduct psychosocial assessments and reassessments for all patient populations, including pediatric, adolescent, geriatric, and general patient populations. Recognize and respond to psychosocial needs of patients and their families, offering crisis intervention and short-term counseling when necessary. Maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, and patients/patient representatives. Ensure effective communication at all levels within the hospital system to coordinate social service/discharge planning activities. Document patient care activities according to current standards and policies, optimizing patient safety and reducing medical/health errors. Provide appropriate resources and support to facility staff. Act as a social services liaison, especially for Advance Care Planning, complex discharge planning, and biopsychosocial assessments. Assess behavioral health patients, arrange evaluations, and coordinate necessary follow-up actions. Participate in team rounds and interdisciplinary collaborations, assisting with discharge coordination for substance abuse treatment centers and mental health programs. Apply clinical judgment to recommend solutions for psychosocial barriers to treatment and discharge planning, coordinating discharge for complex cases and under-resourced patients.

Qualifications Education:

Master's Degree in Social Work (Minimum)

Work Experience:

2 years of clinical social work experience (Preferred) Experience in hospital-based, medical settings, and palliative/hospice care (Preferred) 1 year of MSW internship may be considered. Discharge planning experience (Preferred)

Licenses and Certifications:
