In-person Job Description Summary The Intake Coordinator/Referral Specialist is responsible for all aspects of the patient intake process, including coordinating with the intake team, establishing and maintaining positive relationships with referral sources, responding to requests and concerns, and managing insurance verification and authorization processes. Essential Job Functions/Responsibilities Evaluate hospitalized patients referred for home care. Obtain and review patient history and physical documentation. Identify and verify insurance coverage for home health care services. Secure and document prior authorizations from insurance providers. Assist in obtaining supplemental managed care orders under direction of HMO Intake Manager. Negotiate payment rates (per diem or per visit) for bundled services. Collaborate with hospital discharge planners, utilization review staff, and insurance case managers to facilitate safe, timely discharge. Consult with physicians, nurses, social workers, and other professionals to establish a coordinated plan of care. Verify completion of Face-to-Face documentation. Interview patients, families, and caregivers to assess home situation, needs, and psychosocial factors. Complete referral intake data, hospital course details, and plan of care. Maintain relationships with hospitals and insurance providers, offering information and education on services. Participate in patient care conferences, in-services, and committee meetings as required. Track and report referral statistics regularly. Notify leadership of delays in clinical care. Perform other duties as assigned by supervisors and administrators. Position Qualifications Minimum high school diploma; secretarial or computer background preferred. General knowledge of spelling, grammar, medical records, and office procedures. Familiarity with office equipment and phone systems. Proficiency in computer data entry and software use. Strong communication, organizational, and interpersonal skills. Ability to establish effective working relationships with staff, patients, and referral sources. Professional, positive, and customer-focused demeanor. Reliable transportation and valid driver's license required.
