Intake Coordinator - Home CareGreenmeadows Home Care Santa Ana, CA Full-Time $60,000 - $70,000/ year + Performance Bonus Greenmeadows Home Care is growing! We are looking for an experienced and motivated Intake Coordinator to help manage new patient referrals, communicate with families, and coordinate caregiver staffing. What You'll Do

• Handle incoming patient referrals
• Speak with patients and families to identify care needs
• Complete intake and enter patient information into the EMR
• Assess ADLs, mobility, personal care, supervision, and safety needs
• Coordinate caregiver matching, scheduling, and staffing
• Assist with caregiver recruitment and onboarding
• Communicate with referral sources and the leadership team
• Maintain accurate and timely documentation
• Assist with occasional after-hours/weekend referrals Who We're Looking For
• Experience in home care, healthcare intake, staffing, or scheduling preferred
• Excellent communication and customer-service skills
• Organized and comfortable multitasking
• Comfortable speaking with patients and families
• Familiar with EMR or healthcare software
• Strong follow-up and attention to detail
