• Full-Time Days, 40 hours per week Boulder Community Health is seeking a Physical Therapist
• Outpatient Rehab Services who assumes responsibility and accountability for providing quality patient care through the provisions of physical therapy services including: Assessment of patient functioning, development of treatment programs, provision of patient treatment, and education of the patient and family as it relates to physical/functional abilities; is responsible for working at no less than a 75% productivity rate; must adhere to the APTA Code of Ethics; is responsible for student supervison and aide supervision in the delivery of patient care.

• Current Colorado state license for Physical Therapy.
• BLS certification required by the end of conditional period. Responsibilities
• Evaluative skills: Selects appropriate tests and measures to determine goals and problems, course of treatment and appropriate therapeutic procedures/modalities.
• Selects and establishes physical therapy diagnosis.
• Planning skills: Formulates and maintains an individualized patient plan of care based on functional long and short term goals.
• Implementation skills: Provides treatment using appropriate tools and demonstrates a complete knowledge of patient management.
• Assessment skills: Evaluates patient response to treatment and revises plan of care as indicated.
• Promotes effective communication and teamwork with team members, the hospital and other contacts.
• Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress.
• Initiates and is responsible for problem solving and decision making with relation to patient care.
• Functions as an advocate for the patient within the rehabilitation system.
• Participates in the achievement of department and hospital goals.
