Also introduces families to the home visitation program and/or makes referrals to other appropriate community services. Responsibilities Early identification and assessment of program participants Accepts and screens referrals Interviews and conducts assessments of parents prenatally and/or immediately after birth of child Conduct assessments of all families referred to program from participating community organizations with use of Family Resilience and Opportunities for Growth Scale (FROG or designated standardized tool). Engages community partners and works collaboratively with community organizations involved in identifying prospective program participants. Provide communications with screening organizations and FNWC. Provide training and information to screening organizations as needed Introduce families to designated home visiting program during screening and assessment process. Experience Minimum Bachelor's Degree in Human Services, Medical Field, or Nursing Maturity and experience working with families Experience working with community agencies, including implementing referral processes. Strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value systems and behavioral norms Belief in and be comfortable with the advocating for nurturing, nonviolent discipline of children Knowledge of normal child growth/development and parent-child relationships Ability to relate to families from a strength-based basis Demonstrate motivation and the ability to learn and practice basic supportive skills Experience in working with culturally diverse communities and families with the ability to be culturally sensitive an appropriate Demonstrate good writing and organizational skills Car mobility

Pay:

$27.00 - $30.00 per hour Expected hours: 20.0 per week
