Key Responsibilities Conduct thorough evaluations by reviewing patient histories and performing targeted assessments to establish tailored treatment plans. Communicate evaluation findings clearly to patients and their families, engaging them actively in the goal-setting process to enhance adherence and outcomes. Implement evidence-based treatment protocols refined by patient presentations and adapt these protocols based on continuous assessment of their effectiveness. Collaborate with a multidisciplinary team, including physicians and other healthcare providers, to ensure comprehensive patient care and address any emergent issues requiring modifications to treatment plans. Accurately document all patient interactions and treatment progress in our electronic health records, ensuring transparency and compliance with regulatory standards. Manage time efficiently to meet productivity expectations while contributing to departmental initiatives aimed at enhancing service delivery. Assist in discharge planning, ensuring patients have access to follow-up resources and understand their home exercises clearly. Engage actively in continuing education opportunities to maintain professional development and mentor physical therapy students when applicable. Promote customer service excellence by being adaptable and responsive to patient queries and needs. Patient Population / Client Focus Our clinic serves a diverse array of patients including those recovering from surgery, sports-related injuries, and chronic conditions. Our focus is on personalized care, actively engaging patients in their rehabilitation processes and empowering them with the knowledge to manage their health. Qualifications Bachelor's degree in Physical Therapy from an accredited institution. Active state licensure specific to Connecticut. Current Basic Life Support (BLS) certification from the American Heart Association. A passion for patient care and a positive attitude toward teamwork and collaboration. Preferred Experience A minimum of 2 years of previous experience in an outpatient setting is preferred, where you have developed competencies in performing evaluations and creating treatment plans. Experience in managing a diverse patient population and a strong understanding of rehabilitative techniques and methodologies.
