Job Description
Description We are looking for an organized Intake Coordinator to support referral intake operations for a Long-term Contract position based in Newark, Delaware. This role focuses on coordinating incoming communications, supporting referral workflows, and ensuring timely updates are shared with the appropriate teams. The ideal candidate brings strong administrative judgment, clear communication skills, and the ability to manage multiple priorities in a fast-paced healthcare setting.
Responsibilities:
- Answer and triage incoming calls promptly, directing non-intake inquiries to the appropriate staff members.
- Coordinate referral-related activities by keeping internal teams and external partners informed of next steps and case progress.
- Prepare and distribute daily workflow updates so team members have clear visibility into referral status and activity.
- Record outcomes from visits and related interactions in alignment with established documentation standards and timelines.
- Arrange appointments for referred patients by assigning the appropriate staff and entering all scheduling details accurately.
- Work closely with home care and related care teams to help balance workload and maintain smooth day-to-day operations.
- Obtain required medical documentation by partnering with provider offices, facilities, and other relevant organizations.
- Maintain high service standards when interacting with referral sources, patients, and internal stakeholders.
- Support accurate record handling across intake systems and content management tools used within the department. Requirements
- At least 2 years of experience in intake coordination, client intake, or a similar administrative support role.
- Ability to manage high-volume phone activity while maintaining accuracy, professionalism, and responsiveness.
- Experience scheduling appointments, tracking referral progress, and maintaining organized documentation.
- Strong communication skills with the ability to collaborate effectively across internal teams and external contacts.
- Familiarity with content management systems and electronic record handling processes.
- Comfortable working in a production-driven environment with shifting priorities and established service guidelines.
- Proficiency in handling client application or intake-related workflows with strong attention to detail.
