Our Intake Coordinator plays a critical role in getting patients started on their care journey. Position Summary The Intake Coordinator is responsible for managing the intake and referral process for new patients entering home health services. This role coordinates referrals, verifies information, ensures documentation is complete, and collaborates with clinical and administrative teams to facilitate timely admissions in compliance with Medicare Conditions of Participation, state regulations, and agency policies. Reporting Structure This position reports to the Clinical Manager, Director of Nursing (DON), Administrator, or designated leadership responsible for intake and operations. The Intake Coordinator works closely with clinical staff, referral sources, and administrative departments. Scope of Practice This role is administrative and coordination-focused and does not provide direct patient care or clinical assessments. The Intake Coordinator is responsible for processing referrals, coordinating admissions, and ensuring documentation and eligibility requirements are met prior to patient admission. Essential Job Functions 1. Intake & Referral Processing Receive and process new patient referrals from hospitals, physicians, facilities, and community partners Gather and verify required patient demographic, insurance, and clinical information Enter referral information into the EMR system accurately and timely Coordinate referral follow-up to ensure timely admission decisions 2. Admission Coordination Coordinate scheduling of initial patient visits with clinical staff Ensure all required documentation is obtained prior to admission Communicate admission status updates to referral sources and internal teams Support timely initiation of care in accordance with regulatory requirements 3. Insurance & Eligibility Verification Verify patient insurance coverage and eligibility for home health services Assist with authorization processes as required by payers Communicate coverage limitations or requirements to clinical and leadership teams 4. Communication & Collaboration Serve as the primary point of contact for referral sources during intake process Collaborate with clinical, billing, and scheduling teams to ensure smooth admissions Communicate patient admission status and updates clearly and promptly 5. Documentation & Compliance Ensure intake documentation is complete, accurate, and compliant with agency and regulatory requirements Maintain confidentiality of patient information in accordance with HIPAA Support audit readiness by ensuring proper intake documentation is maintained Qualifications Associate's or Bachelor's degree in healthcare administration, business, or a related field required. Minimum of two (2) years of experience in healthcare intake, admissions, scheduling, or home health operations required. Experience with EMR systems such as Axxess, WellSky, (Kinnser), or similar preferred Strong organizational and communication skills Ability to pass background screening and all compliance requirements Skills We Value Strong communication and interpersonal skills Excellent attention to detail and accuracy Ability to manage multiple referrals and priorities in a fast-paced environment Strong organizational and follow-up skills Knowledge of healthcare intake or insurance processes preferred Professionalism and customer service mindset Physical Requirements Ability to sit and work at a computer for extended periods Frequent use of phone, email, and electronic medical record systems Ability to review and enter patient data accurately Effective communication with referral sources, staff, and leadership Occasional movement within office environment as needed May require occasional travel and exposure to variable environmental conditions during field coordination activities.
