Summary:

Referral coordinator facilitates retrieval of all medical and diagnostic reports and images for new referrals; verifies the accuracy of information obtained and schedules all new patient consults/evaluations.

Responsibilities:

Work closely with nurse navigators, APRN's and physicians to ensure all patient information and documentation is obtained. Download Compact Discs (CD) through the PACS scanning system and transfers images to patient chart. Participate in special projects or activities as requested. Contacts patients and/or providers to obtain all pertinent medical records and verifies the accuracy of all information obtained.
