Duties include, but are not limited to: Complete comprehensive assessments and collaborate with individuals served to develop a care plan that is tailored to their unique needs and goals. Provide assistance and ongoing support for obtaining and maintaining a safe, stable living environment. Provide or arrange for employment, vocational support, and skills training. Provide or assist in arranging for needed support services. Work closely with members of the interdisciplinary team to coordinate care.
