Job Description
We are looking for a detail-oriented a Care Coordination Specialist to join our compassionate care-giving team. In this role, you'll act as a key liaison between patients, providers, and insurance carriers to manage referrals, pre-authorizations, and pre-certifications. You'll work within our Electronic Medical Record (EMR) system to ensure timely and accurate communication with patients, patient's family, staff and providers while providing exceptional support to patients and their families all while, upholding HIPAA regulations, procedures and protocols set forth by the organization is mandatory. If you are adept at managing varying duties and multiple tasks, flexible and able to successfully work with constantly changing priorities, timeframes and deadlines in a fast-paced environment, we'd love to hear from you. Key Responsibilities Serve as a point of contact for referral and authorization requests from providers. Monitor, update, and fulfill incoming requests using the EMR system. Communicate clearly and compassionately with patients, families, providers, and staff. Accurately collect and document information related to insurance pre-authorizations and referrals. Ensure compliance with HIPAA regulations and organizational policies.
