Job Description

Are you an outgoing, organized professional who enjoys building relationships and helping families navigate important transitions in care? A skilled nursing and rehabilitation facility in Lexington, Kentucky is seeking a motivated Admissions Coordinator to join its team! The Admissions Coordinator plays a key role in welcoming new residents and families, coordinating the admission process, developing relationships with referral sources, and helping ensure a smooth transition into the facility. What You'll Do Coordinate the admissions process from initial referral through admission. Respond to inquiries from prospective residents, families, hospitals, physicians, and referral sources. Review and coordinate referrals and gather necessary clinical and financial information. Work closely with nursing, social services, therapy, and the business office to coordinate successful admissions. Verify insurance and payer information and assist with required authorizations and documentation. Schedule and conduct tours for prospective residents and families. Build and maintain positive relationships with hospitals, physicians, case managers, discharge planners, and other referral sources. Follow up with referral sources and prospective residents throughout the admissions process. Complete and maintain accurate admissions documentation. Help monitor census and identify opportunities to grow admissions. Represent the facility professionally in the community and with referral partners. Provide exceptional customer service to residents and families throughout the admission process. Assist with addressing questions or concerns and help ensure a positive experience for new residents and their families. Maintain confidentiality and comply with HIPAA, facility policies, and applicable regulations. Perform other duties as assigned. What We're Looking For Previous healthcare, long-term care, admissions, marketing, sales, or customer service experience preferred. Nursing home or skilled nursing experience is a plus. Strong communication and relationship-building skills. Excellent organizational and time-management skills. Professional, friendly, and confident personality. Ability to work independently while collaborating effectively with an interdisciplinary team. Strong attention to detail and ability to manage multiple priorities. Comfortable communicating with residents, families, healthcare professionals, and community referral sources. High school diploma or equivalent required; additional education or healthcare experience preferred. Why Join Our Team?
