Baton Rouge, LA Job Details Contract 18 hours ago Qualifications Customer communication Full Job Description LA Health and Rehab is looking for an experienced Intake Coordinator to join our team! The Marketing/ Intake Coordinator primary responsibility is to provide exceptional customer service and coordinate all aspects of the admission process, maintain client information in database.
Essential Job Duties:
Responsible for completing intake and inquiry forms for incoming clients. Review packets for criteria nd update intake logs and documentation. Enter required demographic, physician, insurance, authorization, and other information into to client profile. Conduct initial intake assessments and gather necessary information. Review, sign, and obtain client signatures on all of intake documents Schedule patient admission, appointments, and discharges including input patient information in electronic database Provide information and resources to clients as needed. Coordinate care for client with home manager, Resource Coordinator and/or Clinical Director. Assist with planned discharges, referrals, and release of information as deemed appropriate. Answer incoming referral calls. Perform all other duties as assigned by the Executive Director.
Qualifications and Skills:
High School Diploma or GED Minimum of 3 years' experience in medical office required Excellent verbal and written communication skills Must be able to work independently, Possess strong organizational and multitasking skills
