Job Description

Job Title:

Admissions Coordinator Join a healthcare team committed to creating a positive experience for staff and patients.

Requirements:

LVN/LPN desired. Case management/admissions experience required.

Responsibilities:

Handles inquiries, provides tours to interested parties, and interviews patients upon admission. Receives patient referrals, tracks pre-admission referral activity, and coordinates arrangements and confirmation of all admissions. Obtains required signatures on legal consents and insurance forms and ensures receipt of all necessary documentation from referring institutions.
