Job Description

The Intake Coordinator is responsible for managing the admission and intake process for patients entering the inpatient behavioral health facility. The Intake Coordinator serves as the first point of contact for patients, families, and referral sources, coordinating assessments, verifying eligibility, and facilitating a smooth transition into treatment in accordance with LDH regulations, facility policies, and accreditation standards.
Responsibilities:
1. Receive and process all patient referrals, inquiries, and admission requests in a timely, professional, and compassionate manner. 2. Conduct initial screenings and coordinate pre-admission assessments to determine clinical appropriateness and eligibility in accordance with LDH guidelines. 3. Obtain and review clinical, medical, and psychosocial information to support admission decisions. 4. Verify insurance coverage, financial responsibility, and secure necessary pre-authorizations prior to admission. 5. Ensure all required admission documentation is completed, including consents, releases, and LDH-mandated forms. 6. Collaborate with nursing, medical staff, and clinical teams to ensure continuity of care during admission. 7. Communicate clearly with patients and families regarding admission procedures, rights, responsibilities, and facility policies. 8. Maintain accurate and confidential intake records in compliance with
HIPAA, LDH
regulations, and facility policies. 9. Coordinate with external providers, referral sources, and community partners as necessary to facilitate smooth transitions of care. 10. Participate in quality assurance, performance improvement, and compliance activities as required by LDH and facility leadership. 11. Provide 24/7 on-call intake coverage on a rotating basis, if required.
PHYSICAL REQUIREMENTS
The Intake Coordinator must be able to sit, stand, and walk for extended periods; frequently interact with patients and families in crisis situations; and may be exposed to emotionally stressful situations and challenging patient behaviors. The employee must adhere to infection control, safety, and emergency procedures in compliance with LDH and facility policies.
REQUIREMENTS
Skills:
Strong assessment, organizational, and documentation skills. Ability to communicate effectively with patients, families, staff, and referral sources. Proficient in electronic health records (EHR) and Microsoft Office Suite. Knowledge of insurance verification and pre-authorization processes.
Education:
Bachelor's degree in Nursing, Social Work, Psychology, Counseling, Healthcare Administration, or related field preferred. Equivalent experience may be considered.
Licensure/Certification:
RN, LPN, LMSW, LCSW, LPC, or related healthcare license preferred but not always required, depending on facility needs.
Experience:
Minimum of 2 years of experience in behavioral health, admissions, case management, or related healthcare setting preferred.
Knowledge:
Familiarity with LDH regulations, behavioral health licensing standards, and inpatient admission requirements.
