Iron River Care Center is a privately owned, 69-bed skilled nursing and rehabilitation facility that has been successful in the senior care arena for nearly 50 years. You will be responsible to:
Build and maintain relationships with families Conduct tours Manage the referral review process Manage admissions paperwork and process Grow census Build and maintain relationships with hospital accounts Work with nursing and housekeeping staff to ensure a smooth transition for admitting patients Manage and participate in care planning Ensure patients and families are satisfied with the care and services received Coordinate discharge planning and ensure a smooth transition home for patients Other duties as assigned by the facility Administrator Strong leadership skills and the ability to communicate effectively are important characteristics to succeed in this role. Organization, professionalism, and compassion are also integral characteristics for this position. Education and Experience Associates degree preferred Bachelors degree optimal Prefer human services/social work experience with medical background helpful Solid computer skills: Word, Excel, Teams, email, social media platforms, etc. #rewarding #work-life-balance #recognition #team
