General Summary:
In this unique community-based role, you will provide occupational therapy services through two Rivetus programs: "Rivetus Bridge" Mobile Outpatient and "Take Your Therapist Home" Home Health . This position offers a consistent caseload, centralized Oakland County coverage, patient continuity, and the autonomy to build a flexible schedule . Responsibilities include patient assessment, treatment planning and implementation, documentation, care coordination, and supervision of OTAs and aides as assigned.
Your Responsibilities:
Conduct occupational therapy screenings and evaluations to determine patient independence with activities of daily living, including self-care, work, and leisure. Develop and implement individualized treatment plans using adaptations, strengthening, graded activities, and retraining to maximize independence. Provide skilled OT services consistent with AOTA standards and current clinical practice. Treat patients through both Mobile Outpatient and Home Health programs within a centralized Oakland County territory. Provide direct patient care in accordance with clinical and productivity standards. Coordinate patient visits and manage your schedule with a high degree of clinician autonomy. Supervise, instruct, assign, and review the work of OTAs and aides as assigned. Complete accurate and timely documentation supporting quality, continuity of care, and reimbursement. Participate in patient care conferences, CQI activities, continuing education, in-services, and program development as requested. Participate in company meetings and activities supporting effective departmental operations.
Required Qualifications:
Successful completion of an accredited Occupational Therapy program. Current licensure as required by the State of Michigan . New graduates are encouraged to apply.
