We'll Build Around It. We're not asking you to keep your entire week open hoping shifts become available. We're looking for dependable CNAs who can tell us, "This is the day I can consistently give you." During that availability, you'll travel between scheduled hospice patients providing hands-on personal care in private homes, Assisted Living, Memory Care, and other community settings. For someone who wants to keep another job, attend school, manage family responsibilities, or simply work fewer days, it can be a great way to keep using your CNA skills without rebuilding your entire life around work . What You'll Do Provide scheduled hospice CNA visits throughout the Western Twin Cities Metro Area Assist with bathing, showering, grooming, dressing, toileting, and personal hygiene Help with mobility, transfers, repositioning, and activities of daily living Provide comfort-focused care according to each patient's care plan Encourage independence whenever appropriate Observe changes and communicate concerns to the nursing team Provide companionship and reassurance to patients and families Complete accurate and timely visit documentation Collaborate with hospice nurses and the interdisciplinary care team Bring the CNA Experience You Already Have We're interested in CNAs from Assisted Living, Memory Care, Skilled Nursing, Long-Term Care, Hospitals, Home Health, Hospice, Palliative Care, Private Duty, and In-Home Care . Hospice experience is preferred, but not required. You do need an active Minnesota Certified Nursing Assistant (CNA) certification, previous hands-on care experience, a valid driver's license, reliable transportation, and the confidence to work independently between patient visits.
