Summary:
This Employee is responsible for organizing, marketing, coordinating, and managing the Facility's Admissions process to provide potential clients with Facility-related written information, Facility tours, personal meetings, assessments, communications, and follow-up; implements and coordinates effective marketing programs and exhibit a sincere desire to work with the elderly. Additionally, Responsible for identifying and providing each Resident's social, emotional and psychological needs for the continuing development of the Residents' full potential; assists with Discharge Planning for Transfer and Discharge; must exhibit a sincere desire to work with the elderly and communicate courteously with Employees, Residents, and others.
Duties and Responsibilities:
- Cultivates communications with hospital Social Services, Discharge Planners, families, and other referral sources; obtains approval of Administrator or designee prior to each Admission
- Provides Facility tours; provides in-hospital visits/assessments, communicates with family members of prospective Residents
- Manages the Admissions process by always maintaining updated census data and Facility information
- Manages the inquiry process professionally and timely with appropriate follow-up
- Effectively manages occupancy levels and census/case mix enhancement, utilizing bed management principles
- Develops Social History, Social Assessment, Care Plan to identify problems, needs, set realistic goals, problem resolution and/or needs upon Admission, compliant with State and Federal Regulations
- Documents Progress Notes for Resident's Care Plan when necessary, within Company Policy time frames
- Assists Residents and family members with discharge planning and discharge process
- Promotes communication assistance and support for family members and Residents; aids with TDD phones and/or other communication assistance for impaired Residents
- Attends public relations activities as requested by the Administrator and assists with community education regarding aging, Facility services and other related information Paid Time Off (PTO) including: Vacation pay Paid sick leave Insurance benefits including: Group Health insurance Dental insurance
