Responsibilities In addition to the key responsibilities associated with the roles and scope of Nurse Care Manager or Care Manager-Social Worker, the team leader key responsibilities include: Serves as clinical expert for staff within a designated area and acts as first level consultant for staff within designated team to address patient care issues, discharge barriers, etc. Advises staff appropriately and includes departmental leadership as necessary. Serves as point of contact and mentor for members of the designated team. Monitors census daily to ensure appropriate staffing and census distribution within the designated team. Schedules and conducts team meetings/huddles (weekly) Manages PTO and holiday staffing for designated team. Provides for educational support to residents, attending, and new medical students for respective designated assigned areas. Minimum Requirements Masters of Social Work degree from a CSWE accredited school of social work is required with minimum two years case management experience or A BSN in nursing from an accredited college or university required with minimum 2 years case management experience is required. If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.
