What You'll Do As a member of our Home Health & Hospice team, you will: Deliver skilled nursing care in patients' homes Monitor patient conditions and communicate updates to the healthcare team Assist with implementing individualized care plans Educate patients and families on health conditions and care needs Provide comfort-focused care and support to hospice patients and their loved ones Promote patient independence, safety, and overall well-being What We're Looking For Qualified candidates will have: Current, active Nebraska Licensed Practical Nurse (LPN) license Current CPR certification Strong clinical assessment and communication skills Compassionate, patient-centered approach to care Excellent organizational and time-management abilities Ability to work independently while collaborating effectively with a team Commitment to providing exceptional service and care Why Brookestone Home Health & Hospice?
