Community Support Services Intake Coordinator Non-Exempt/ Full- Time
REPORTING RELATIONSHIP
Reports to the Director of Community Support Services SUMMARY Responsible for screening all applicants for eligibility, accepting new referrals, initiating applications, and providing assistance to applicants in obtaining services of the Community Support Services programs. These programs provide services, including home care, to primarily low-income seniors and people with disabilities who wish to age in place in the community.
RESPONSIBILITIES
Within the limits of the agency's policies and procedures, an Intake Coordinator is responsible and has commensurate authority to perform the duties listed below and is directly accountable to the Director of Community Support Services. Performance of these duties requires extensive and regular supervision.
Duties included are:
- Receiving all initial referrals, screening for eligibility, and documenting calls on contact log.
- Completing SCS for all JACC applicants.
- Mailing appropriate applications to applicants and providing follow up.
- Referring callers to other agencies for services provided outside of FCS.
- Adding applicants to referral and wait lists.
- Receiving applications and reviewing for completeness.
- Following procedures specific to each program as it relates to applications.
- Maintaining databases of referrals and wait lists for each program.
- Assisting with administrative duties related to
JACC, CG, & SRCP.
- Performing other administrative tasks as requested by Director.
- Access to clinical record information is limited to the extent required to perform responsibilities effectively & efficiently.
- Responsible for maintenance of client records as required by grantors.
- Attends biweekly department meetings as required by Director
WORKING RELATIONSHIPS
Works closely with the Director of Community Support Services, social workers, and agency staff.
WORKING ENVIRONMENT
This is an in-office position. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The majority of work is of light to medium duty, which requires standing, sitting, bending, speaking, hearing, manual dexterity, and visual acuity.
BACKGROUND/EDUCATION/TRAINING/EXPERIENCE
Associate degree in a health related or behavioral science field required. Preferably one year of experience working with the elderly or physically disabled in an institutional or community setting. Have demonstrable work habits, such as accountability, punctuality, and respect for clients. Have the ability to accept and utilize supervision. Excellent communication and organizational skills; ability to focus on details. Competency in communication, e.g. reading, writing and speaking Must be computer literate and have experience with Word and Excel. Pay:
$22.00 per hour Expected hours: 35.0 per week
