The Intake Coordinator is responsible for the registration process of clients as they enter the REAL House Inc. facility. Your responsibilities as an intake coordinator include gathering patient records, taking medical histories, and obtaining any insurance information from the patient. This position requires a strong knowledge of appropriate personal and professional boundaries, a consistently positive attitude, and the ability to deal with difficult, uncertain, and stressful situations in a positive manner.
JOB QUALIFICATIONS
A combination of training, education and experience that is equivalent to one of the employment standards above and that provides the required knowledge and abilities. Review all incoming referrals and conduct initial screening to ensure they meet the defined admission criteria Contact referring agencies for medical information and documentation Conduct informational and screening interviews with patients and their families as part of the admission process Enter client-specific data into the computer system Track all referrals and follow up as needed. Participates in other duties as assigned by the Clinical Director and Program Director Close working relationship with referral sources, the Director of Patient Services, Coordinator, Human Resources and Marketing staff Respond to the referral sources and patients' needs in a professional and creative manner Demonstrate commitment, professional growth and competency Promote Agency philosophy and administrative policies to ensure quality of care Excellent assessment, organizational and communication skills Strong written and oral communications skills. Ability to work flexible hours Ability to work in and adapt the culture of a multi-cultural, community based organization Good inter-personal skills, ability to relate well with persons from a variety of socioeconomic and cultural groups Commitment to maintain confidentiality Understanding of the Agency's mission and culture and the ability to maintain appropriate boundaries with staff and clients in all circumstances Proficiency in computer skills, especially using Outlook, Word, Excel and customized databases.
Pay:
$50,000.00 - $60,000.00 per year
