Now Hiring - Intake Coordinator The Intake Coordinator is responsible for, but not limited to: Completing initial inquiry forms for all potential home health admissions and forwarding completed forms to the appropriate clinical staff, Director of Clinical Services, and/or Administrator. Working to convert qualified referrals into admissions whenever possible. Notifying the Administrator when referrals do not convert to admissions. Tracking all inquiries and referrals daily and providing a weekly report to the Administrator. Great New Career Opportunity in Home Health! Working collaboratively with appropriate staff to ensure all required written or verbal consents are obtained.

Education:
Associate degree in a related field preferred.
Professional Experience:
Five to eight years of related experience preferred. Experience in an intake, admissions, healthcare, or related capacity preferred. Valid driver's license and reliable transportation required. Must be willing to travel. Excellent communication, interpersonal, and organizational skills.
