Patient Navigator-Emergency Services An Emergency Department Patient Navigator is a trained, culturally sensitive health care worker capable of providing support and guidance helping patients transition from the emergency department to outpatient follow-up care or home. Provide support and guidance throughout the hospital and community. Help navigate doctors' offices, clinics, hospitals, outpatient centers, insurance and payment systems, and other components. Assisting with SBIRT Screening MH Care Coordination Monitoring and following up on discharge call backs Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services.

Qualifications:

High School Graduate or GED 2 years experience in healthcare, hospital or clinic environment.
