Full-Time Join a Team That Makes a Difference Buffalo Homecare is seeking a compassionate, organized, and motivated Intake Coordinator to join our growing team. In this role, you will be the first point of contact for new clients and referral sources, helping individuals access the care and support they need. What You'll DoWelcome New Clients Serve as the first point of contact for clients, families, and referral sources. Guide new clients through the onboarding process with professionalism and compassion. Answer questions about services and provide clear information regarding eligibility and care options. Manage Referrals & Admissions Receive and process referrals from hospitals, healthcare providers, case managers, and community organizations. Follow up promptly on incoming referrals. Verify client information and ensure all required documentation is completed accurately. Coordinate Care Services Work closely with scheduling, nursing, and care management teams to coordinate the start of services. Assist with arranging initial caregiver schedules based on client needs and availability. Help ensure a smooth transition from referral to active services. Maintain Accurate Records Enter and update client information in agency systems. Maintain organized and compliant documentation. Ensure all records meet agency, state, and federal requirements. Support Agency Growth Build strong relationships with referral partners and community organizations. Assist with problem-solving and resolving issues that may arise during the intake process. Help create a positive experience for every client and family. What We're Looking ForQualifications High School Diploma or GED required. Associate's or Bachelor's degree in Healthcare Administration, Social Work, Business, or a related field is a plus. Previous experience in home care, healthcare, customer service, admissions, intake, scheduling, or case coordination preferred. Experience working directly with clients, patients, or families is highly valued. Skills Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to multitask and prioritize in a fast-paced environment. Detail-oriented with strong problem-solving skills. Compassionate, professional, and service-focused attitude.
